If you are in need or know of someone that is in need, an initial referral can be made by any of the following: Potential recipient, friend, family member, medical professional (social worker, nurse, child life specialist, doctor, etc.) Upon receipt and review of your submitted referral form, a member of our organization will directly contact the parents in need and provide an application form to be completed.
